Secureone is an authorized fingerprint provider for the Illinois Healthcare Worker Registry and can facilitate fingerprinting for the Illinois Department of Public Health.

• Register your employee with the IDPH Portal and send them with form printed from site after registration
• Come to our Crestwood office for fingerprinting and submission – Card scan services are available for out of state applicants
• Cost is $40 – Corporate discounts available, please call if creating a billing account
• No Appointment is necessary
